Why do you need a form? You already know the requirements...

Send scores, date, tournament, your name, GNAS number, club, bow style and £5 (for the extra 100 points ). Easy as pie.

Thanks Grant I've emailed him - I wasn't sure Bob was still doing it as there is a different name on the Spring Article - also don't know what the cut off dates are but I'm sure Bob will let me know and will probably need to mail something, but I can get Bob's address from Archery UK. Who needs a silly form!
