MB / GMB Claim Form

Does anyone know where I can get hold of the MB / GMB claim form?
I've had a good look on the GNAS website and can't find it. I also emailed GNAS but as expected got no reply.
I thought it may have been published in Archery UK but can't find it in the issues I have (not got the newest one yet though).

You might get one, unannounced, in the post if you've emailed GNAS. It is a paper form, rather than an electronic one. I think they want to avoid too many people having access to it for whatever reason.

I've got one I can probably scan in tonight, if you can't get ahold of one before then.

The form is yellow and I wonder if that's what they want to keep about it.

As they generally send them with the application form for the UK masters, if you know of anybody who was MB last year, and is not this year they may be able to give it to you. If you email GNAS remember to say whether you want a target or field form.

No point emailing GNAS. The girl in the office who knows how to open the email is on holiday. Ring them instead. It takes up more of their time, so they get less done, so that must be logical.

I wanted a copy of the insurance certificate recently. I emailed and waited, emailed again (thought they could just scan it and email back to me saving postage and envelopes) finally my deadline was drawing near, so I phoned and they posted a photocopy to me which I received the next day.
Cost to the gnas office, one person to read 2 emails, one person to answer the telephone and deal with my enquiry, use of the photocopier, envelope and postage. If the girl comes back off her hols, I may end up with 3 copies, so you can multiply all this by 3.

Alternative, scan the document once, keep it on the hard drive, email to any one requesting a copy. Cost to GNAS Office, one person to deal with one email. Overall saving 12 person jobs, 3 envelopes, 3 postage.

No wonder we never have any money

I phoned GNAS for a copy of the MB/GMB form. It was posted out within a couple of days of my call.
